Military Directory Opt Out Form

Greetings 11th and 12th-grade students of Jasper County School District (or School Name). In accordance with Section 8528 of the act titled Elementary and Secondary Education Act of 1965 (ESEA), Jasper County School District is required to provide names, addresses, and phone numbers of high school students to institutions of higher education as well as military recruiters.

If the high school student aged 18 or older or parent of a student under 18 chooses not to share such information, they will need to complete an opt out form. The form can be retrieved from the school and must be completed and returned on or before January 13, 2023. If you have any questions, please contact your child’s school."
