Student Vaccination Survey

Hello JCSD Family,

JCSD is partnering with Coastal Carolina Hospital to make the Pfizer vaccine available to all students 12 years old and older.

We will host a student vaccination event ahead of the 2021-2022 school year.

The purpose of this survey is to get a count for how many students are interested in receiving the vaccination. Please fill out a survey for each student that you would like to receive the vaccination. You can access the survey at the link below. The survey will be available until Friday, July 14, 2021.
