Student Transportation Request Form

 
In order to initiate and/or receive pupil transportation services, parents and/or guardians are adjured to submit a request and/or change of service form. Please fill in the requisite information to request new service and/or a change of service for your school. You can submit a request for transportation and/or a change to service to your school at any time. Once a parent and/or guardian fulfills the initial steps, a designee at JCSD will communicate and/or disseminate your child's bus route information via My Bus Route - https://jcsd.mybusplanner.com/ .
To access your child's bus route and/or change of service:

Click on Parent Portal Icon
Create an Account - Enter your email address.
Create a Password - Password Requirements: one upper case letter, one lower case letter, one special charter, one number and it must be at least 8 characters
All requests for transportation and/or a change of service are imposed within 72 hours of notification. If you can't create an account and/or sign in, select the Contact tab, fill in the form and a JCSD designee will contact you within 24 hours.
